APPLICATION REQUIREMENTS

We appreciate this opportunity to be of service to you in obtaining a real estate loan. Please complete the enclosed forms and return them with the following items that apply to your loan request.

ITEMS NEEDED

INCOME VERIFICATION if borrower(s) receive salary or wages:
  • Two consecutive PAYCHECK stubs for each borrower
  • Last two years W-2 all employers

INCOME VERIFICATION if borrower(s)are self-employed or receive commission:
  • Last two years complete Federal Tax Returns (signed and dated)
  • Last two years corporate and/or Partnership Federal Tax Returns (if applicable)
  • Copy of 2 years business license or letter on CPA letterhead (if applicable)
  • Current Profit and Loss Statement (if applicable)

ASSET VERIFICATION & PROPERTY INFO IF A PURCHASE LOAN
  • Most recent 3 months consecutive bank statements for checking, savings, money market, stock, retirement and/or 401k accounts (please include ALL pages of statements)
  • Complete copy of purchase contract
  • Escrow information (if available)
  • Gift letter including proof of donors ability (if applicable)
  • Landlord name and address or 12 months cancelled checks (front and back) to establish rent history
  • If the subject property includes a septic tank, well, private road, or is located in a flood zone, it is very important to bring this to our attention at the time you submit your completed application.
ASSET VERIFICATION & PROPERTY INFO IF A REFINANCE
  • Most recent 3 months consecutive bank statements for checking, savings, money market, stock, retirement and/or 401k accounts (please include ALL pages of statements)
  • Letter stating reason for refinance
  • Please indicate if the property is currently listed for sale or has been during the last 12 months.
OTHER PERTINENT INFORMATION WE NEED (IF APPLICABLE):
  • Copy of Divorce decree including property settlement, alimony & child support requirement
  • Names of town home or condo complex and homeowner association: include address and phone number
  • Copy of rental agreement(s) for each investment property
  • Copy of any note from which you are currently receiving income
  • Documentation of pension or Social Security
  • Copy of note (all pages) of the first mortgage loan (required for all 2nd mortgage or equity loans)
  • Impound account? (Do you wish to pay your taxes & insurance monthly with your loan payment?) Yes ____ No ____

FEES REQUIRED
  • Check made payable to: Credit Services for $25 for a credit report
  • Check made payable to: Appraisal Service for Appraisal fees: $325 to $600 if owner-occupied or $425 and up if non-owner-occupied. (Fees depend on sales price or loan amount-call us for the exact amount)
OTHER POSSIBLE FEES
  • Check payable to: PFS MORTGAGE for $ 20.00 for desktop approval.
